A Proctoscope Top Open (Operative) is a specialized surgical instrument designed to provide unobstructed transanal access to the anal canal and rectum. Unlike standard diagnostic proctoscopes that feature a fully closed cylinder, an operative open-top model includes a longitudinal U-shaped cutout or slot along the top of the sheath. This design allows surgical tools (like forceps, ligators, or lasers) to easily manipulate tissue while preserving maximum visual fields. 

Core Operational MechanismAn operative proctoscope relies on a multi-part assembly to facilitate tissue entry and access: 

Insertion with Obturator: The hollow speculum sleeve is fitted over a solid inner core called an obturator. This core features a smooth, rounded olivary tip that expands the anal sphincter gently without tearing the mucosal walls. 

1. Obturator Withdrawal: Once positioned inside the anal canal, the provider extracts the obturator. The remaining hollow sleeve keeps the canal walls retracted.
2. Surgical Intervention: The open-top slit or beveled distal end isolates the treatment site (such as internal hemorrhoids or rectal polyps). Instruments enter through the main bore and manipulate tissue freely out of the top slot.
